FCC Purchase Request Form

Please use this form to submit your general purchase requests (FCC debit card or reimbursement).

Important: You must get approval before buying any item. The FCC will not reimburse purchases made without pre-approval.

FCC Faculty Grant Requests

Faculty: please use this form to submit faculty grant requests for 2026-27, to be reviewed by the RFSA administration and SIT. If approved, grants will be funded by the FCC to the maximum extent possible.

PBL & Pathway Requests

Faculty: please use this form to submit requests for PBL & Pathway material purchasing or reimbursement in 2026-27. All requests will be reviewed by lead STEAM staff prior to FCC purchase.
